Background: Reactive oxygen species play an important role in schsistosomal granuloma formation and disease progression. IL-10 may downregulate cell-mediated immunity. Few data are available about oxidative stress and the immune response of cases of (HCV) coinfected with Schistosoma mansoni. Aims & Methods: To determine pattern of serum IL10, MDA levels as a markers of oxidative stress and total antioxidant in patients with combined HCV and schistosomiasis and their role in disease progression. 60 patients were classified into: Group 1: 20 patients with schistosomal hepatic fibrosis; group 2: 20 patients with HCV; group 3: 20 patients with combined HCV and schistosomiasis. In addition to 20 controls. Serum levels of IL 10, malonaldehyde (MDA), total anti oxidant capacity (TAC) were determined using chemical methods & HCV RNA titer by RT/PCR, liver biopsy were determined. Results: Serum levels of IL-10 were significantly elevated in combined group more than chronic HCV patients (p < 0.05). Serum levels of IL-10 did not correlate with the different laboratory parameters, ishak score or HCV-RNA viral load for all patients (p > 0.05). Serum levels of MDA were significantly elevated while TAC levels were significantly decreased in all groups (p < 0.001). Serum MDA had significant positive correlation with Ishak score fibrosis in both HCV & co infection groups (p = 0.01 and 0.002 respectively). Mixed group had necro-inflammatory scores significantly higher > HCV alone (p = 0.03). Serum TAC showed negative correlation with Ishak score in the mixed group (p = 0.018). In stepwise multiple regression analysis the MDA was the predictor for liver fibrosis in both HCV and co-infected groups. Conclusion: Schistosomiasis triggers a Th2 type immune response and triggers an increase in MDA.
